Tyrvaya Patient Savings Offer Terms and Conditions

Valid only for patients with private (commercial) prescription drug insurance and a valid prescription for Tyrvaya® (varenicline solution) nasal spray. This Patient Savings Offer ("Offer") is not valid if the patient is enrolled in any government sponsored healthcare plan or program including, but not limited to, Medicare, Medicare Advantage, Medicare Part D, Medicaid, Tricare, Veterans Affairs, or any other federal or state health care program. This Offer is not valid for non–insured/cash-paying patients, or where a patient's private prescription drug insurance plan covers or reimburses the entire cost of the drug. Patient must be 18 years of age or older to use this Offer. This Offer is not valid where prohibited by law and is valid only in the United States.

This Offer must be presented to the pharmacist with a valid prescription to participate in this Offer. By using this Offer, eligible, commercially insured patients with coverage for Tyrvaya pay as little as $10, up to a savings maximum of $275 for each 30-day prescription, and as little as $10, up to a savings maximum of $825 for each 90-day prescription. Patient is responsible for any out–of–pocket costs once the monthly savings limit is reached. Eligible, commercially insured patients awaiting a coverage decision from their insurance plan pay $10 for a 30‑day prescription, for up to 1 fill. While during this 1 fill, if the patient receives coverage, the patient will transition to the pay as little as $10 offer mentioned above for their next refill. This Savings Offer expires on 12/31/2024.

The value of this Offer is exclusively for the benefit of patients and is intended to be credited towards patient out‑of‑pocket obligations and maximums, including applicable copayments, coinsurance, and deductibles. Patient may not seek reimbursement for the value received from this Offer from other parties, including any health insurance program or plan, flexible spending account, or healthcare savings account. Patient is responsible for complying with any applicable limitations and requirements of their health plan related to the use of this Offer. Patient should not use this Offer if their health plan prohibits use of manufacturer copay assistance. This Offer is not health insurance and may not be combined with any other offer, including any third-party rebate, coupon, or free trial. When using this Offer, the patient certifies that he/she understands the rules, requirements, and these Terms and Conditions, and has responded truthfully to all questions when activating this Offer. Proof of purchase may be required. No other purchase is necessary. Oyster Point reserves the right to change or discontinue this Offer at any time, without notice.
